Recovery Alberta seeks an Outreach Worker I to join the AMH Supported Housing team. You will conduct outreach assessments, develop treatment plans with clients and clinicians, and connect clients with community and government supports.
The role involves teaching daily living skills, observing client progress, and collaborating with a multidisciplinary team. A vehicle and valid driver's license are required for field visits and home assessments.
Reporting to the Manager, the Outreach Worker I is a member of the AMH Supported Housing team. The team functions collaboratively to provide treatment, rehabilitation, support services and case management functions for clients with severe and persistent addiction and mental health presentations. Team members collaborate with the client, family and other programs and agencies to provide quality, client-driven, culturally appropriate and integrated services.This position supports clients to live in the community in a variety of supportive housing environments: conducts outreach assessments, determines outreach service goals and develops outreach treatment plans in collaboration with clients and clinicians, while facilitating client connections with community and government supports and advocating for clients with these external service providers. The OW supports clients with building ADL and iADL skills, medication observation and teaching, establishing financial, food and clothing, transportation and social/recreational supports. Collaboration with the multidisciplinary team includes reporting subjective and objective changes in the client’s mental health presentation and ability to function independently. Additional responsibility includes ongoing evaluation of client progress/functioning and adjusting the outreach treatment plan as required. As a part of the role, the OW may be required to provide outreach crisis stabilization support for clients, various de-escalation techniques and linking clients with the appropriate supports (EMS, Police and Crisis Team, etc.).This position requires staff to perform home visits to clients’ homes. Please Note: This position requires a vehicle for business use. Possession of a valid driver’s license and access to a motor vehicle in good repair for work-related travel is required.
As an Outreach Worker, you will work as a member of a multidisciplinary team, supporting clients to stabilize in their current environments as an extension of their clinical care service. As an Outreach Worker, you will conduct outreach assessments, determine outreach service goals and develop outreach treatment plans in collaboration with clients and clinicians. You will facilitate client connections with community and government supports and advocate for clients with these external service providers. You will focus on establishing housing, financial, food and clothing, transportation and social supports for clients.You will be responsible for providing education and coaching support related to daily living skills and activities (attending appointments, money management, searching for jobs, grocery shopping, etc.), observing and reporting changes in the clients' behaviours, participation and ability to function independently to the interdisciplinary team. You will evaluate client progress and functioning and make adjustments to the outreach treatment plan as required.As a part of your role as an Outreach Worker, you may be required to provide outreach crisis stabilization support for clients, exercising various de-escalation techniques and linking clients with the appropriate supports (EMS, Police and Crisis Team, etc.). You also may be required to enter clients' residences and assist clients to transfer in and out of vehicles, care facilities and residences.
Completion of diploma from a post-secondary program in a related field.
Minimum 2 years’ experience within the last 3 years working with adults with severe and persistent mental health and addiction issuesExperience in conducting psychosocial assessments, treatment planning, crisis intervention, and medication managementExperience working with multidisciplinary teams and community agenciesKnowledge of relevant community resources and legislationAbility to be self-directed and exercises independent decision making, and critical thinking skillsHigh degree of flexibility as well as the ability to work independently in an outreach capacityAbility to work in stressful, demanding and changing conditionsExcellent interpersonal and communication skills with the ability to effectively maintain collaborative working relationships with members of the multidisciplinary teamExcellent time management and prioritization skills, with a demonstrated ability to function effectively within a dynamic environmentDemonstrated problem solving and conflict resolution skillsActively integrates patient and family centered care principles within own practice standardsExcellent verbal and written communication skillsGroup facilitation experienceBasic knowledge of computers and software applications (i.e. Microsoft Office)
Recent community-based mental health outreach experienceKnowledge of Mental Health Legislation especially related to Community Treatment Orders
