WoodGreen Community Services in Toronto is seeking a Personal Support Worker to join our community care team. The role is 35 hours per week, Monday to Friday, 7:00 am to 3:00 pm, onsite at 570 O'Connor Dr.

Salary is $24.37 per hour and includes a bargaining unit position with 1 vacancy. You will provide essential home and cluster care services to clients, adhere to program policies, document accurately, and work closely with supervisors and co-workers to promote safety, dignity, and independence

Formación

• Minimum one (1) year experience Providing personal care to seniors and people living with disabilities.
• PSW certificate and (if applicable) NACC certificate.
• Ability to perform light to moderate lifting and physical tasks.
• Strong communication and teamwork skills.
• Willingness to transfer between program locations.

Responsabilidades

• Provides direct service to clients based on program needs and within scope of practice.
• Adheres to health and safety policies and procedures.
• Clock in/out using Alayacare Mobile Application.
• Prepare meals, menu adjustments, and grocery management.
• Document accurately in client records and program files.
• Maintain professional appearance and interactions with clients and staff.
• Coordinate with Supervisor, Scheduler and other staff to ensure consistent service delivery.
• Assist with transport, home visits, and client support as required.
