Job summary

VHA Home HealthCare (VHA) in North York is seeking an Occupational Therapist to provide adult-focused home and community-based services. You will assess, plan, and implement OT interventions aligned with client goals and professional standards, collaborating with the care team to optimize independence.

The role supports flexible scheduling and community-based care across settings, with emphasis on evidence-based practice, documentation, and client-centered outcomes.

Qualifications

• Degree in Occupational Therapy from a recognized school of Occupational Therapy.
• Member in good standing with the College of Occupational Therapists of Ontario.
• Valid Driver's License with access to a vehicle.
• Compassionate and empathetic; self-driven with ability to work independently.
• Skilled in caseload management and electronic documentation.
• Able to provide virtual care.

Responsibilities

• Assesses and implements occupational therapy interventions in accordance with professional practice guidelines and standards.
• Implements client interventions based on assessment information, client input and care plans.
• Ensures services are best practice, evidence-based, and measured for outcomes.
• Coordinates delivery of care through ongoing communication with the client's team.
• Documents assessment findings, treatment plans, progress and discharge summaries as required.
• Supports safe client care through continuous quality improvement.

Pay Range
Estimated annual earning potential: $80,000–$130,000.

Actual earnings may vary based on availability and service volumes

Key Areas of Accountability

• Assesses and implements occupational therapy interventions in accordance with professional practice guidelines and standards established by VHA and the perspective regulatory college.
• Implement client interventions based on assessment information, client input and in conjunction with the client's integrated care plan.
• Ensure that services provided are best practice, evidence-based and evaluated through the routine use of valid and reliable outcome measured.
• Ensure coordinated delivery of care through ongoing communication with all members of the client's team regarding plans and schedules for interventions.
• Document all assessment findings, treatment plans, progress and discharge summaries of interventions as required b the profession, referral source and the organization.
• Supports safe client care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.

Required Qualifications:

• Degree in Occupational Therapy from a recognized school of Occupational Therapy.
• Member in good standing with the College of Occupational Therapists of Ontario.
• Valid Driver's License with access to a vehicle
