As an Occupational Therapist , you will assess and determine patient care needs and eligibility, provide accessand referrals to community services, and engage with patients, caregivers and other health care practitioners.

What will you do?
The Occupational Therapist completes assessments, develops treatment plans, organizes, provides and directs occupational therapy treatment and educational programs for Ontario Health atHome Champlain clients, their families and caregivers in accordance with the standards of the College of Occupational Therapists of Ontario.
Primary Responsibilities :
Completes reassessments to review patient's progress and treatment goals and reports to Care Coordinator and other members of the health care team in order to ensure continuity of care and determine if other resources and referrals are required.

Salary Range : $44.480 / hr to $51.340 / hr
