As an Occupational Therapist, you will assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers, and other healthcare practitioners.

What will you do?

The Occupational Therapist completes assessments, develops treatment plans, organizes, provides, and directs occupational therapy treatment and educational programs for Ontario Health atHome Champlain clients, their families, and caregivers in accordance with the standards of the College of Occupational Therapists of Ontario.

Primary Responsibilities
  • Completes assessments of clients in various settings: private homes, schools, senior residences, long-term care facilities, group homes, and performs analysis based on objective, subjective, and functional findings.
  • Establishes realistic, measurable, and time-limited treatment goals and develops treatment plans.
  • Ensures occupational therapy treatments are initiated with informed consent, in accordance with the Health Care Consent Act.
  • Provides direct treatment/support and education to informal caregivers, school personnel, and other health professionals involved.
  • Participates in case conferences and consults with other team members to foster holistic care.

Reassesses patients to review progress and treatment goals, reports to Care Coordinator, and determines if other resources or referrals are needed.

  • Plans, in collaboration with the patient, family, and Care Coordinator, for the termination of services when goals are achieved or no longer required.
  • Completes assessments and recommendations for adaptation aids, assistive devices, and equipment, respecting the Ontario Assistive Devices Program where appropriate.
  • Maintains accurate patient records in line with the College of Occupational Therapists of Ontario standards and organizational policies.
  • Acts as a resource by providing educational programs as requested.
  • Attends and actively participates in team and staff meetings.
  • Maintains current professional knowledge and skills.
  • Prioritizes occupational therapy services according to organizational policies.
Additional Details

Team/Department: Allied Health

Hours of work: 8:30 am to 4:30 pm, Monday to Friday

FTE: 1.0

Length of Temporary Assignment: October 25th, 2025

Salary Range: $44.480/hr to $51.340/hr

Reports To: Manager, Patient Services

Affiliation: OPSEU

Qualifications
  • Degree or diploma in Occupational Therapy and current registration with the College of Occupational Therapists of Ontario
  • Related professional experience, typically over two years in an Occupational Therapy role
  • Experience in community-based settings is an asset
  • Current CPR-C certificate
Preferred Knowledge and Skills
  • Knowledge of healthcare system and Ontario Health atHome Champlain's role within it
  • Knowledge of relevant healthcare legislation and organizational policies
  • Strong interpersonal, communication, and customer service skills
  • Ability to manage difficult conversations effectively
  • Ability to work independently and in a team
  • Effective planning, organizing, and documentation skills
  • Proficiency with computer software and Microsoft Office
  • Valid driver’s license and access to a vehicle
  • Fluency in English; French is an asset
  • Vulnerable Sector Check completed within the last six months
Work Schedule & Benefits

Monday to Friday, 8:30 am to 4:30 pm, with a hybrid work model combining in-office and telework options.
