Job description

POSITION SUMMARY

Our client is a leading stand-alone organization in Pediatric Health, with over 400 staff and physicians dedicated to helping children and youth thrive. We are currently seeking an Occupational Therapist – Home & Community Care.

The Occupational Therapist provides services in homes, schools, and other settings, conducting clinical assessments, developing treatment plans, and setting goals in collaboration with patients, families, and interdisciplinary teams, adhering to the standards of the College of Occupational Therapists of Ontario.

This role supports the School-Based Rehabilitation Services (SBRS) Program and in-home therapy services as needed. Additionally, some assessment and treatment for children from birth to 18 years may be provided in the Development & Rehabilitation department. Services may be delivered virtually or at client locations across Eastern Ontario.

JOB DESCRIPTION
  • Complete assessments of patients in various settings, analyzing findings objectively, subjectively, and functionally.
  • Establish realistic, measurable, and time-limited treatment goals; develop treatment plans; ensure treatments are initiated with informed consent in accordance with the Health Care Consent Act.
  • Provide direct treatment, support, and education to caregivers, school personnel, and healthcare professionals involved.
  • Participate in case conferences and consult with team members to promote holistic care.
  • Perform reassessments to monitor progress, report to the Care Coordinator, and determine if additional resources or referrals are needed.
  • Plan for service termination collaboratively when goals are met or services are no longer required.
  • Assess and recommend adaptations, assistive devices, and equipment, ensuring compliance with the Ontario Assistive Devices Program where appropriate. Maintain accurate documentation and patient records per standards and policies.
  • Serve as a resource by providing educational programs to employees, service providers, school personnel, and the community.
  • Attend team and staff meetings; maintain professional knowledge and skills; prioritize services according to policy.
QUALIFICATIONS

  • Graduate from an accredited Occupational Therapy program (Essential)
  • Registered with the College of Occupational Therapists of Ontario (C.O.T.O.) (Essential)
  • Minimum two years' pediatric experience (Preferred)
  • Experience in community-based settings (Preferred)
  • Authorization for the Assistive Devices Program of Ontario (Preferred)
  • Current CPR-C certification (Preferred)
  • Effective communication skills, both oral and written (Essential)
  • Excellent interpersonal skills (Essential)
  • Ability to work inter-professionally (Essential)
  • Strong time management and organizational skills (Essential)
  • Ability to prepare and facilitate therapeutic groups (Essential)
  • Valid driver’s license and access to a vehicle (Essential)
  • Bilingual in English and French (Essential)
