- Conduct comprehensive and physical assessments using interviews, standardized tests, and/or guided observation
- Identify factors influencing occupational performance
- Develop client-centered treatment plans collaboratively with patients, physicians, and other interprofessional healthcare providers
- Provide individual and group interventions designed to maximize activities of daily living performance
- Refer to and partner with community agencies and providers to support care
- Educate and assist patients in identifying appropriate community resources, programs, and government services
- Establish effective relationships and collaborate with interdisciplinary healthcare professionals
- Maintain documentation in accordance with college standards
- Participate in program and resource development with the Therapy team
- Manage a diverse lifespan caseload, including dementia, chronic disease, chronic pain, and repetitive upper-extremity musculoskeletal injuries
- Work across multiple TVFHT locations in St. Thomas and Strathroy, on-site and providing home care when necessary
Requirements
- Bachelor or Master of Science in Occupational Therapy
- Preferred experience across diverse fields of practice
- Current registration with the College of Occupational Therapists of Ontario
- ADP Authorized
- Excellent written and verbal communication skills
- Solid understanding and experience with adult group learning principles
- Ability to teach self-management techniques and evoke participation in a group environment
- Comfort with, or willingness to become familiar with, virtual meeting technology
- Experience facilitating an online program considered an asset
- Excellent organizational, time management, critical thinking, and problem-solving skills
- Ability to establish positive interpersonal relationships and work effectively as a team member
- Demonstrated commitment to evidence-based practice
- Knowledge of and ability to mobilize community resources
- Flexibility in adapting to varying daily caseload needs
- Experience using EMR, Microsoft Word, Excel, Outlook, and Teams
- Reliable transportation for travel between sites
- Successful completion of a criminal background check and medical screening
- Social Insurance Number and, in some cases, a valid work permit required to be eligible to apply
