Spectrum Health Care seeks an Occupational Therapist to deliver ministry-funded community-based services in clients' homes or via virtual care. Conduct comprehensive home assessments, develop intervention plans, and coordinate with clients and caregivers to improve safety and function.
The role requires OT registration in Ontario, strong communication, and the ability to work independently in a community setting.
Provide ministry-funded, community-based occupational therapy services in clients' homes and/or through virtual care
Conduct occupational therapy assessments using interviews, standardized tools, and informal assessment techniques, including:
Develop and implement evidence-based intervention plans in collaboration with clients and caregivers
Assign, supervise, and monitor occupational therapy assistants as appropriate
Educate clients and caregivers and connect them with relevant community agencies and resources
Complete funding applications when required
Maintain accurate, timely, and professional documentation, including:
Registration in good standing with the College of Occupational Therapists of Ontario (COTO)
Ability to work independently within a community-based model
Strong communication, documentation, and clinical reasoning skills
Experience in community practice is an asset, but new graduates are welcome
Valid G or G2 driver’s license with access to a reliable vehicle for community travel
Proficiency with computer applications, including MS Office Suite and proprietary clinical software
