Job summary

Joseph's Home Care is seeking an Occupational Therapist to support adults in a community-based home setting in Ontario. You will assess function, develop personalized rehab plans, and educate clients and families on goals, exercises, and equipment.

Join an interdisciplinary team, supervise OTA/PTA staff, and contribute to care planning for clients with complex medical needs in their homes.

Qualifications

• Bachelor’s or Master’s Degree in Occupational Therapy (Master’s preferred).
• Registered with the College of Occupational Therapists of Ontario (COTO).
• Current BCLS/HCP certification.

Responsibilities

• Conduct holistic assessments of cognitive, physical, perceptual, and emotional function.
• Develop and implement individualized treatment plans.
• Educate clients and caregivers on therapy goals, exercises, and adaptive equipment.
• Collaborate with interdisciplinary teams for discharge planning and care coordination.
• Supervise OTA/PTA staff and mentor students as needed.
• Support clients with complex medical conditions in their homes.
• Maintain infection control protocols and ensure safe use of equipment.
• Document care activities and maintain accurate client records.
• Assist with scheduling, referrals, and program development.
