Job summary

Liquid Personnel is seeking an Occupational Therapist to provide a wide range of interventions for adults and children in the client's Rutland area. The role focuses on enabling independence and safety at home, supporting service users to live life to the fullest.

You will assess a broad caseload, deliver DFG assessments, and coordinate equipment and major adaptations while collaborating with health and social care colleagues to ensure seamless service delivery.

Qualifications

• Qualified Occupational Therapist with HCPC registration.
• Experience working across adults and/or children’s services.
• Strong knowledge of DFG assessments and major adaptations.
• Excellent communication, problem-solving, and risk assessment skills.

Responsibilities

• Assess adults and children with disabilities, arranging equipment and adaptations to maximise independence and safety.
• Deliver DFG assessments – experience and knowledge of Disabled Facilities Grants is essential.
• Undertake complex risk assessments for moving and handling, and develop effective management plans.
• Involve service users and carers in assessments, support planning, and accessing Direct Payments.
• Identify innovative solutions including assistive technologies, providing demonstrations and ensuring safe use.
• Maintain accurate case records – completing timely assessments, monitoring/reviewing support plans, and updating systems to support service delivery and data collection.
• Work collaboratively with colleagues and wider health and social care professionals to deliver seamless support.
• Engage in professional development and actively contribute to developments within Adult Social Care.
