Job summary

Liquid Personnel is seeking an experienced Occupational Therapist to join their remote assessment service. You will triage referrals, complete telephone/video assessments, prescribe equipment and minor adaptations, and contribute to prevention‑focused work across Adult Social Care.

This fast-paced role requires strong clinical reasoning, risk management, and digital communication skills, with occasional home visits to support complex cases.

Qualifications

  • Experience in Adult Social Care, community OT, hospital discharge or reablement settings (Essential).
  • Experience managing complex cases and risk in fast-paced environments (Essential).
  • Experience completing remote assessments (Desirable).

Responsibilities

  • Triage referrals and identify priority, risk and required OT intervention.
  • Conduct remote assessments via telephone and video call.
  • Prescribe equipment and minor adaptations per service guidelines.
  • Provide preventative advice and problem‑solving support to adults, carers and professionals.
  • Escalate safeguarding concerns per statutory procedures.
  • Maintain accurate and timely case records.
  • Collaborate with community health, housing, social work and safeguarding partners.
  • Participate in supervision, MDT meetings, and service development activities.

Job description

Liquid Personnel is seeking an experiencedOccupational Therapistto join their remote assessment service. You will triage new referrals, complete telephone/video assessments, prescribe equipment and minor adaptations, and contribute to prevention‑focused work across Adult Social Care. This is a fast‑paced role requiring strong clinical reasoning, risk management, and digital communication skills.

What will your responsibilities be?
  • Review referral information to identifypriority, risk, and required OT intervention.
  • Carry outremote assessmentsvia telephone and video call.
  • Providepreventative advice, guidance, and problem‑solving support to adults, carers, and professionals.
  • Escalatesafeguarding concernsin line with statutory procedures.
  • Apply strongfunctional questioningand clinical reasoning to assess needs remotely.
  • Prescribestandard equipment and minor adaptationsin accordance with service guidelines.
  • Makedefensible, evidence‑based clinical decisionsthat reflect risk, strengths, and need.
  • Maintainaccurate and timely case records.
  • Contribute toprevention‑focused work, including equipment reviews, moving & handling advice, and self‑management strategies.
  • Work collaboratively with community health, housing, social work, reablement, voluntary sector teams, and safeguarding partners.
  • Participate in supervision, MDT meetings, training, and service‑development activities.
  • Promote asolution‑focused and strengths‑based culture.
Qualifications & Experience:
  • Experience working inAdult Social Care,community OT,hospital discharge,reablement, or similar settings (Essential).
  • Experience managingcomplex cases and riskin fast‑paced environments (Essential).
  • Experience completingremote assessments (Desirable).
