Job summary

The Care Company is seeking an experienced Registered Nurse to deliver complex in-home care to pediatric, adult, and geriatric clients across the Greater Toronto Area. You will work autonomously in clients’ homes with clinical oversight from the care team.

Mandatory credentials include CNO registration, current CPR/First Aid, TB testing, and COVID-19 vaccination status, plus a valid Ontario driver’s licence to travel between client homes.

Qualifications

  • The RN must be actively registered in good standing with the College of Nurses of Ontario (CNO).
  • Current CPR and First Aid certification within the past 12 months.
  • Valid WHMIS 2015 certification within the past 12 months.
  • Negative TB test within the past 12 months.
  • Proof of full COVID-19 vaccination or exemption as per policy.
  • Clear Criminal Record Check with Vulnerable Sector Screening within 12 months.
  • Valid Ontario driver’s licence and access to a reliable vehicle for GTA travel.
  • Flexibility for days, evenings, overnights, weekends and holidays.

Responsibilities

  • Provide one-on-one home care including bathing, grooming, and activities of daily living.
  • Administer medications per physician orders and document in eMAR.
  • Monitor vital signs and report abnormalities to care coordinator or physician.
  • Support oxygen therapy, G-tube feeding, and suctioning per care plan.
  • Assist with mobility and transfers using lifts or manual methods.
  • Perform ROM and light physical therapy exercises as directed.
  • Document changes in client condition and escalate as needed.
  • Maintain compliant documentation and adhere to infection control protocols.

The Care Company is looking for an awesome, self-driven and highly organized and detail-oriented Registered Nurse (RN) to assist us with complex care paediatric, adult, and geriatric clients in the home who wish to live at home.

The following Requirements are MANDATORY:

  • Active registration and in good standing with the College of Nurses of Ontario (CNO) as a Registered Nurse (RN)
  • Current CPR and First Aid certification, completed through a recognized training provider within the past 12 months.
  • Valid WHMIS 2015 certification, issued or renewed within the past 12 months.
  • Negative TB test (2-step or 1-step with prior documentation), completed within the past 12 months.
  • Proof of full COVID-19 vaccination, or exemption in accordance with the Ontario Human Rights Code.
  • Clear Criminal Record Check with Vulnerable Sector Screening, issued within the past 12 months.
  • Must possess a valid Ontario driver’s licence and access to a reliable vehicle for travel to client homes and schools across the Greater Toronto Area (GTA).
  • Must be available to work a variety of shifts, including days, evenings, overnights, weekends, and holidays as required.
  • Must be available for last-minute shift coverage, including on-call response, to support the continuity of client care.
  • Must be able to communicate effectively in English, both orally and in writing, for documentation and communication with clients and care teams.
  • Must be comfortable and capable of providing care to both male and female clients of varying ages and care needs.
  • Demonstrated experience delivering in-home care and support in school-based settings for both pediatric and adult clients.

Must have hands-on clinical experience providing:

  • Pediatric and adult tracheostomy care, including cleaning, suctioning, and tube changes as per physician orders
  • Pediatric and adult ventilator care, including management of AIRVO humidification systems and ventilator alarms
  • Gastrostomy (G-Tube), Jejunostomy (GJ-Tube), and Nasogastric (NG-Tube) feeding and care for pediatric and adult clients
  • Oral, nasal, and tracheal suctioning for secretion management in both pediatric and adult clients
  • Seizure monitoring and management protocols for pediatric and adult clients with known seizure disorders
  • Ability to identify, document, and elevate any changes in condition to the supervising nurse or care coordinator.
Duties and Responsibilities:
  • Provide one-on-one personal care to clients in their home environment, including full assistance with bathing (bed baths, showers, perineal care), oral hygiene, grooming, toileting, incontinence care, dressing, and feeding, in accordance with the client’s care plan and scope of practice for a Registered Nurse (RN)
  • Administer medications orally, topically, subcutaneously, or via G-tube, as per CNO scope and documented medical orders; document all administrations in the electronic Medication Administration Record (eMAR).
  • Monitor and assess clients’ vital signs (blood pressure, pulse, temperature, respirations, oxygen saturation), document findings, and report any abnormal readings or concerns to the designated Care Coordinator or physician.
  • Support clients with medical needs such as oxygen therapy (monitoring levels, changing tanks, checking saturation), G-tube feeding (preparing and administering formula, flushing lines), and suctioning as per training and client-specific protocols.
  • Safely assist clients with mobility and transfers using mechanical lifts (Hoyer, sit-to-stand) or manual assistance, following prescribed mobility plans and ensuring proper body mechanics and safety procedures.
  • Perform prescribed range of motion (ROM) and physical therapy exercises under direction of a physiotherapist or care plan, and assist with ambulation inside the home or outdoors if required.
  • Observe and document any changes in the client’s physical, cognitive, or emotional condition, including changes in skin integrity, alertness, behaviour, and breathing patterns; elevate concerns as needed.
  • Maintain accurate and timely documentation of all care provided, including progress notes, incident reports, and communication logs, in compliance with company and regulatory standards.
  • Use and dispose of Personal Protective Equipment (PPE) such as gloves, masks, gowns, and eye protection as per infection prevention and control protocols and Public Health Ontario guidelines.
  • Comply with organizational policies regarding infection control, client safety, privacy, and professional conduct; report all hazards or incidents to the supervisor immediately.
  • Competitive rates of pay ranging from $43.25-$50.75/hour (depending on shift)
