Job summary

Pinecrest-Queensway Community Health Centre is seeking a Nurse Practitioner for the West Ottawa HART Hub. The NP will provide integrated withdrawal management, addiction medicine, episodic primary health care and mental health services to individuals aged 16 and older within a trauma-informed, harm-reduction framework.

The position is regular part-time, with coverage primarily on Thursdays, Fridays and weekends, reporting to the WOHH Director.

Qualifications

• Interdisciplinary collaboration and care coordination within a trauma-informed, client-centred framework.
• Ability to assess, diagnose, treat, and manage acute and chronic health concerns within NP scope.
• Knowledge of harm reduction, addiction medicine, withdrawal management and primary care considerations.

Responsibilities

• Provide trauma-informed, culturally safe care to WOHH clients.
• Assess, diagnose, treat, prescribe and monitor substance use, physical health and mental health concerns within NP scope.
• Deliver withdrawal management, addiction medicine, episodic primary health care and mental health services.
• Identify health risks, develop care plans, provide health education, and support recovery goals.
• Collaborate with the interdisciplinary team and external partners to ensure coordinated care.
• Maintain documentation and participate in chart audits and quality improvement.
