Bring your clinical expertise and passion for providing quality and compassionate care to a challenging and rewarding role as a Nurse Practitioner in the Patient Assessment and Transition to Home (PATH) unit at Peace Arch Hospital in White Rock, B.C. The PATH unit is an inpatient hospital-based program in Fraser Health where Nurse Practitioners assume the role of the Most Responsible Provider (MRP) for patients. As part of this rewarding MRP role you will have the opportunity to participate in compensated on-call work rotations to assist the unit remain open 24/7, 365 days a year. NPs in PATH units support and empower medically stable alternative level of care (ALC) designated patients with complex discharge requirements and facilitate successful transitions home to thrive in the community.

Peach Arch Hospital
As a community hospital in Fraser Health’s integrated network of care, the role of Peace Arch Hospital is to provide primary, secondary, and some specialty services, such as 24/7 emergency, general medicine, surgery, maternity, inpatient, and outpatient care, with some services requiring a referral while others can be accessed directly. Peace Arch Hospital has a capacity of 171 acute care beds as of July 2018. It provides care and services primarily to local community residents, and, to a limited extent, residents of the surrounding communities. In addition to service delivery, Peace Arch Hospital is a teaching hospital affiliated with UBC’s family medicine and psychiatry residency programs.

The Nurse Practitioner is responsible and accountable for the comprehensive assessment of patients/clients/residents including diagnosing diseases, disorders, and conditions. The Nurse Practitioner initiates treatment including health care management, therapeutic interventions and prescribes medications in accordance with the statutory and regulatory standards, limits, and conditions, and employer policies and procedures. The Nurse Practitioner provides professional guidance to other health professionals and practices autonomously and interdependently within the context of an interdisciplinary health care team, making referrals to specialist physicians and others as appropriate.
The position collaborates with patients/clients/residents and other health professionals to identify and assess trends and patterns that have implications for patients/clients/residents, families and communities; develops and implements population and evidence based strategies to improve health and participates in policy-making activities that influence health services and practices. The position participates in peer review and self review to evaluate the outcome of services at the patient/client/resident, community and population level.
Current practicing registration as a Nurse Practitioner with the British Columbia College of Nurses and Midwives (BCCNM).
Recent relevant clinical nursing experience.
Demonstrates the leadership practices of the Fraser Health Leadership Framework of Clear, Caring and Courageous and creates the conditions for people to succeed.
