Job summary

The Barrie and Community Family Health Team (BCFHT) is a large Ontario-based family health team focusing on patient-centred, team-based primary care. This Nurse Practitioner (MRP) role involves serving as the Most Responsible Provider for an assigned patient panel, within an integrated care model with RNs, dietitians, pharmacists and other professionals.

Qualifications

  • Current registration with the College of Nurses of Ontario (CNO).
  • Completion of a recognized Primary Care Nurse Practitioner program: NP-PHC.
  • Masters degree in Nursing is an asset.
  • Member of NPAO is an asset.
  • Completion of an accredited controlled-substance course according to CNO standards.
  • Experience with independent health care assessment and patient management.
  • Ability to work extended hours, including after-hours clinics.
  • Experience working with inter-professional teams and strong communication skills.
  • Experience with EMRs (Accuro) and MS Office (Excel, Word, Outlook).

The Barrie and Community Family Health Team (BCFHT) is one of the largest Family Health Teams in Ontario. Our team

includes physicians, registered nurses, nurse practitioners, dietitians, pharmacist, diabetes and respiratory educators,

mental health counsellors, social workers, occupational therapists, foot care nurses, and administrative staff.

Our team is committed to improving access to comprehensive, patient-centred, team-based primary health care. Our approach supports self-management, emphasizes health promotion and disease prevention, and enhances the care of

individuals living with chronic conditions through programs that are well connected with local health and community

services.

The BCFHT has an existing position for a:

NURSE PRACTITIONER (MRP)

LOCATION: Barrie, ON

POSITION DESCRIPTION

The Nurse Practitioner (MRP) will work within an integrated care model, collaborating with other interdisciplinary health professionals. The Nurse Practitioner will provide comprehensive, patient-centered primary care across the

lifespan, with an emphasis on health promotion, disease prevention, and patient education. Working to their full scope of practice, the Nurse Practitioner will act as the Most Responsible Provider (MRP) for an assigned panel of patients.

NPs on our team work collaboratively with a variety of other health care practitioners including Registered Dietitians, RNs, Pharmacist, Diabetes and Respiratory Educators, and Mental Health Counsellors.

PRIMARY RESPONSIBILITIES
  • Provide primary health care to patients in accordance with standards set by governing body, and that is aligned with the overall strategic direction of the family health team
  • Provide health promotion, disease prevention, acute episodic care, chronic disease management, and palliative care to patients
  • Enhance access to assessment, diagnosis and treatment within the community and/or target population
  • Consult as appropriate, with members of the interdisciplinary team, acute care, after hours medical clinics, community care system, and specialist physicians to assess, plan, implement, evaluate and coordinate a patient's plan of care and treatment
EDUCATION, QUALIFICATIONS AND SKILLS
  • Current registration with the College of Nurses of Ontario (CNO)
  • Completion of a recognized Primary Care Nurse Practitioner program: NP-PHC
  • Masters Degree in Nursing, considered an asset
  • Member of NPAO, considered an asset
  • Completion of an accredited controlled substance course according to CNO standards
  • Independent health care assessment and patient management skills
  • Ability to work extended hours, including after hours clinics
  • Experience working with inter-professional teams
  • Demonstrated communication skills and problem-solving abilities
  • Experience with electronic medical records (Accuro an asset) and MS Office (Excel, Word, Outlook)
ADDITIONAL JOB REQUIREMENTS
  • Must adhere to all BCFHT Policies and Procedures
  • Must be legally entitled to work in Canada
  • Must be able to travel within the community
COMPENSATION

The BCFHT offers a competitive benefits package including health, dental, and pension plan (HOOPP) for those eligible. We do not use AI to screen, assess or select applicants for the position. The salary range for this position is $113,124-$128,613/year ($62.16/hour -$70.67/hour).
