Job summary

Get Well Family Health Team in North York, ON is seeking a Nurse Practitioner for a part-time role (20-30 hours per week) to deliver comprehensive primary care across the lifespan. The NP will focus on health promotion, disease prevention, chronic disease management, and episodic care while collaborating with physicians and the interdisciplinary team.

Opportunities to attach patients as MRP or share care with an existing PCP are available.

Qualifications

• Must be registered in Ontario as NP-Extended Class with the CNO and hold a Primary Care NP certificate.
• Minimum of 2 years’ experience in primary care or related clinical settings; new graduates with strong interest will be considered.
• Experience with pediatrics, adults, seniors; strong communication and teamwork are essential.

Job description

We are seeking a Nurse Practitioner, part-time available for 20-30 hours/week.

Get Well Family Health Team is a collaborative, interdisciplinary primary care team serving patients in North York. Our team includes family physicians, nurse practitioners, nurses, social workers, dietitians, psychotherapists, physiotherapist, chiropractor, naturopathic doctor, and other health professionals who work together to provide comprehensive, patient-centred care. We are committed to improving access, quality, and health outcomes for our community.

Role Description

The Nurse Practitioner (NP) is an integral member of our Family Health Team, working within the full scope of practice to provide primary health care services to patients across the lifespan. The NP will focus on health promotion, disease prevention, chronic disease management, and episodic acute care, while collaborating closely with physicians and other interdisciplinary health professionals. There are opportunities for directly attaching patients as MRP, or shared-care with an existing primary care provider. The GWFHT is a clinical teaching site for the University of Toronto Temerty Faculty of Medicine.

Key Responsibilities

• Provide comprehensive primary care, including assessment, diagnosis, treatment, and follow-up of patients.
• Prescribe medications, order diagnostic tests, and interpret results in alignment with NP scope of practice in Ontario.
• Support chronic disease prevention and management programs (e.g., diabetes, hypertension, COPD).
• Perform physical exams, PAP test, well baby visits, immunizations, diabetes care, preventative health visits.
• Deliver health promotion, education, and counseling to patients, families, and caregivers.
• Fill out various medical, workplace, government forms for patients.
• Documentation for medical, statistical, and administrative purposes.
• Provide intake assessment to help navigate patients to receive appropriate care within the family health team.
• Liaise with healthcare providers and facilitate referrals within the Family Health Team and to secondary/tertiary care services.
• Review and manage results and consultation reports.
• Navigate the health system for patients and families.
• Collaborate in the development and delivery of group programs and quality improvement initiatives.
• Participate in interdisciplinary case conferences and care planning.
• Apply reflective practice to evaluate and improve nursing care quality.
• Maintain accurate, timely, and complete documentation in the electronic medical record (EMR).
• Perform cross-functional and other assigned responsibilities.
• Foster positive public relations and collaborative, family-centered care.
• Support clinic administration, including clinical coaching, inventory management (supplies, medications, vaccines, PPE), and crash cart oversight.
• Contribute to the development of policies, protocols, and clinical pathways to support evidence-based practice.
• Take on additional initiatives that contribute to the growth of the health team.

Qualifications

• Current registration in the Extended Class (RN-EC) with the College of Nurses of Ontario (CNO).
• Primary Care Nurse Practitioner Certificate
• Member in good standing with the Nurse Practitioners’ Association of Ontario (NPAO) (preferred).
• Valid CPR HCP certification.
• Minimum of 2 years’ experience in primary care, community health, or a related clinical setting (new graduates with strong interest in primary care will be considered).
• Experience with pediatrics, adults, seniors care, chronic disease management, women's health
• Demonstrated ability to work effectively within an interdisciplinary team.
• Excellent clinical assessment, diagnostic, and decision-making skills.
• Strong communication and patient engagement skills.
• Strong interpersonal, organizational, and multitasking skills; detail-oriented and team-focused.
• Ability to take direction from multiple sources (physicians, nurses, team leads).
• Flexible availability, including some evenings and Saturdays.
• Able to work across multiple clinic locations within North York
• Proficiency with electronic medical records (EMRs), computer softwares, and comfort with virtual care platforms.
• Fluent in English, additional languages reflecting the community (e.g., Cantonese, Mandarin, Farsi, Korean and Tagalog) are an asset.
• Valid vulnerable sector police record check completed within the last year.
