Brightshores Health System in Owen Sound is seeking a Nurse Practitioner for the Medicine department.
The NP will work as an autonomous advanced practice nurse, collaborating with the Hospitalist group to develop care plans for patients on the Transitional Care Unit and within the acute medicine program.
Reporting to the Program Manager of the Medical Department, the Nurse Practitioner works as a self-directed advance practice nurse. The NP will work collaboratively within the interprofessional practice model as well as in partnership with the Hospitalist group. The NP will use advanced knowledge, skill, and judgement when providing care to patients. They will work with the interprofessional team to develop the plan of care for patients on the transitional care unit., and within the acute medicine program. This includes identifying solutions for health-care issues and leading a robust discharge planning process for patients within the Transitional Care Unit - supporting ALC patients while waiting for Long Term Care. The Nurse Practitioner will collaborate with physician colleagues to support patient care throughout their hospital stay on the Medicine unit. The Nurse Practitioner provides patient care in accordance with legislative acts and frameworks guiding Nurse Practitioner practice including those from the Brightshores hospital guidelines, policies and medical directives, College of Nurses of Ontario (CNO), the Regulated Health Professionals Act, 1991 (RHPA), the Public Hospitals Act, 1990 (PHA), the Nursing Act, 1991, and professional practice standards. Within the scope of these acts and frameworks, the Nurse Practitioner will demonstrate competency to autonomously diagnose, order and interpret diagnostic tests, prescribe pharmaceuticals and perform procedures as appropriate.
