Job description

Reporting to the Director of the Regional Cardiac Care Program at Southlake Health, the Nurse Practitioner is an integral member of the Regional Cardiac Care Program (RCCP). Southlake’s RCCP is one of the four largest cardiac regional programs in Ontario and provides regional cardiac care within York Region, Simcoe, and South Simcoe.

Responsibilities:

1. The Heart Failure Nurse Practitioner (HFNP) provides high quality, safe, effective, and evidence-based care in partnership with patients and families.
2. Uses advanced clinical knowledge and clinical practice expertise to promote optimal health and high quality of care. In this role, the HFNP works to full NP scope, is autonomous in accordance with the College of Nurses of Ontario (CNO), Standards of Practice: Nurse Practitioners.
3. The HFNP will work collaboratively with the interdisciplinary team to optimize Heart Failure Care delivery across multiple settings, including inpatients and outpatients.
4. Active participation in management of hospitalized patients with acute decompensated heart failure.
5. In the outpatient clinic, HFNP provides follow-up for post-inpatient heart failure admissions and outpatient niche programs such as home IV Lasix, home inotrope, and palliative heart failure care in collaboration with the Advanced Heart Failure Cardiologist.
6. The HFNP will provide leadership and assist with professional development of interdisciplinary team members in the Cardiac and Medicine Programs through mentoring and participating in formal and informal education.
7. Development and evaluation of policies that promote quality care by implementing evidence-based practices.
8. Inform the development of key clinical pathways to improve the quality of care, reduce risks, increase patient satisfaction, and increase the efficiency in the use of resources.
9. Evaluate patient outcomes through the development of appropriate metrics that would monitor the program’s quality and best practice.
10. The HFNP will provide cross coverage to the heart function clinic and medical cardiology as required.
