Summary Of Duties

The Nurse Practitioner (NP) is an advanced practice role designed to contribute to the quality of patient care through practice, research, and education. The NP promotes wellness and optimal health, providing high-quality care through advanced clinical and theoretical knowledge and skills. The NP practices in alignment with Sunnybrook’s Mission, Vision, and Values, and adheres to the professional standards of nursing practice as defined by the hospital and the College of Nurses of Ontario.

The role involves coordinating the clinical management of patients with new, chronic, or advanced heart failure in the outpatient setting. This includes short-term medical optimization and long-term follow-up for patients requiring close monitoring, management, and evaluation.

The NP collaborates with patients, families, and other healthcare providers to deliver safe, high-quality, patient-centered care. Additionally, the NP works with the Patient Care Manager, Clinical Educator, and other Advanced Practice Nurses to develop, implement, and evaluate best practices, clinical pathways, and quality improvement initiatives. The NP also evaluates patient outcomes using appropriate metrics to monitor program quality and adherence to best practices.

Qualifications/Skills
  • Current College of Nurses of Ontario Certificate of Competence
  • Master’s degree with Nurse Practitioner Certification
  • Demonstrated clinical expertise in cardiac surgery or cardiology preferred
  • Experience in geriatrics and managing chronic conditions
  • Current B.C.L.S.
  • Membership in R.N.A.O.
  • Previous experience as a Nurse Practitioner
  • Program planning and evaluation skills
  • Leadership qualities, innovation, and creativity
  • Excellent interpersonal and communication skills
  • Expertise in conflict management
