Job description

MissionHires is partnering with a modern transitional care facility in West Bedford, Nova Scotia to hire full-time Nurse Practitioners (MRP role). In this on-site, weekday position, you’ll independently assess, diagnose, prescribe, and lead interdisciplinary teams to deliver evidence-based, patient-centered care for adults transitioning from acute care back to home or community. Responsibilities
  • Serve as the Most Responsible Health Care Provider (MRP) for adult patients in a transitional care setting
  • Independently assess, diagnose, prescribe, and treat within legislated NP scope of practice
  • Lead on-site clinical teams and provide direct supervision and clinical expertise
  • Collaborate closely with interdisciplinary teams (restorative care professionals, nurses, allied health)
  • Develop and implement evidence-based, patient- and family-centered care plans focused on safe transition home or to community
  • Manage a patient panel of approximately 22–23 patients per NP
  • Contribute to a supportive, collaborative, and outcomes-driven care environment
  • Uphold regulatory, documentation, and quality standards aligned with organizational and union guidelines
Requirements
  • Master’s Degree in Nursing with Nurse Practitioner Certification
  • Eligible for registration with the Nova Scotia College of Nursing (NSCN)
  • 2–3 years of experience as a Nurse Practitioner preferred (acute or rehabilitative care ideal)
  • Comfort and proficiency providing hands-on, on-site patient care
  • Advanced clinical decision-making and diagnostic skills
  • Demonstrated leadership and shift supervisory capabilitiesProven ability to work effectively within an interdisciplinary team environment
  • Availability to work Monday–Friday, 08:00–16:00; no current on-call requirement
  • Note: Pediatric NP backgrounds cannot be considered due to licensing limitations
