Employment Status: Full-Time for 12 months and potentially 18 months, pending leave of absence
Hours of Work/Schedule: 35 hours per week; 8:30 to 4:30 or 9 to 5 on Monday to Friday.
Benefits: Family Medical Plan Benefits, the HOOPP, pension plan and more if the Employment option is offered, or no benefits with a higher hourly rate to an Incorporated NP.
Position Reports To: Senior Manager, Integrated Care
Primary Work Location: 1-2 days per week in our client’s south Oshawa Clinic with free parking and 3 – 4 days per week doing home visits with a team member, with mileage reimbursement.

The Locum Qualifications • experience working collaboratively with interdisciplinary teams • experience working with patients with cognitive impairment and dementia related symptoms • experience in a GAIN team, Regional Geriatric Program, Long Term Care preferred • Assets include professional development in areas such as U-First, PIECES, Gentle Persuasive Approach, Mental Health First Aid, Dementia Foundations • Able to drive with access to a vehicle to conduct home visits, as needed (mileage is reimbursed) • Computer proficiency is required and experience with Telus PS Suites is an Asset
