Description
The Ambulatory Programs Nurse Practitioner (NP) provides high-quality, patient-centered care within the Ambulatory Programs, including but not limited to the Ambulatory Day Clinic, Respiratory Clinic, Internal Medicine Clinic, Wound Clinic, Chemotherapy, and Hemodialysis. This role involves assessing, diagnosing, and managing acute and chronic conditions, delivering direct patient care, coordinating with internal medicine physicians, interdisciplinary teams, community partners, and contributing to program development and patient education initiatives. The NP plays a critical role in optimizing patient outcomes and enhancing the overall experience of care in an ambulatory setting.
As the Ambulatory Programs NP, you will work closely with Internal Medicine physicians to provide longitudinal care for outpatients, including in-person and virtual reassessments, ordering and reviewing investigations, prescriptions (including renewals), and titration of medications for patients with CHF, COPD, Cirrhosis, Diabetes Mellitus, and other complex comorbidities.
Additional responsibilities include providing primary care for internal medicine patients without a primary care provider and assisting with patient concerns, including arranging follow-up when required. This position is an opportunity to increase your breadth and depth of practice as you will be supported by the Internal Medicine physicians working at LWDH.
