The Nurse Practitioner (NP) reports to the Executive Director and collaborates with residents, families/caregivers, and the healthcare team in developing, implementing, and evaluating residents’ care plans. They also provide leadership and mentorship to team members, enhancing their knowledge, assessment skills, and ability to care for residents.
Responsibilities :
- Diagnose residents, order and interpret diagnostic tests, and prescribe medication and treatments.
- Provide direction and leadership to RNs, RPNs, PSWs, and other care team members within legislative standards.
- Complete nursing documentation and assist residents with move-in processes.
- Demonstrate passion for quality dementia care and capturing positive moments.
- Collaborate with attending physicians.
- Contribute to a person-centered team approach.
- Ensure medication reconciliation as needed.
- Implement resident-focused programs, policies, and procedures with the Director of Nursing and Quality Team.
- Provide education and in-services to the clinical team.
- Liaise with residents, families, and outside agencies as required, respecting the Residents' Bill of Rights.
- Perform any other duties as assigned.
Qualifications :
The qualifications needed to join our family are:
- Registered Nurse Practitioner in good standing with the College of Nurses of Ontario or equivalent in another Canadian jurisdiction with independent practice rights in Ontario.
- Post-graduate education and experience in gerontology (recommended).
- Specialty certification in gerontological nursing (recommended).
- Experience working as a Nurse Practitioner.
- Experience in long-term care, providing empathetic elderly care.
- Leadership and supervision experience in healthcare.
- Ability to utilize team skills to maximize individual and collective potential.
- Must have a recent vulnerable sector screen, TB test within 6 months, and three supervisory references.
