The Guelph Family Health Team is seeking a Nurse Practitioner for a 0.8 FTE, 6–12 month contract in Ontario. You will work in collaboration with physicians, patients, families and external partners to provide comprehensive, team-based care.
In this role you will apply advanced clinical skills to diagnose and manage acute and chronic conditions, participate in quality improvement, research activities, and contribute to policy and education initiatives within a multidisciplinary setting.

Hourly Rate: $67.93 – $70.68
Reports to: Manager, Clinic Operations
The Guelph Family Health Team is a not-for-profit, interdisciplinary primary care organization providing comprehensive, team-based care to the community of Guelph and surrounding areas. Our team works collaboratively to improve patient access, health outcomes, and system sustainability through innovative and integrated primary care services.
The Nurse Practitioner will function in collaboration with members of the Guelph Family Health Team, Physicians patients, families, and external partners. The Nurse Practitioner successfully functions in an expanded role demonstrating expert skill to formulate clinical decisions and appropriately manage acute/chronic illness and promote wellness. The NP utilizes a comprehensive theoretical knowledge base and an advanced level of clinical competence in caring for patients, involving collecting and interpreting data, determining a medical diagnosis, and treating complex responses of individuals and families to actual or potential problems. The NP performs quality improvement and administrative activities and participates in policy and procedure development.
Other key components of the role include performing research activities; delivering, coordinating, and participating in education and professional development activities/programs; performing activities to further develop and strengthen the profession; acting as a resource and serving as a consultant to individuals and group within the professional community.
