NURSE PRACTITIONER (NP) (Full-Time – Independent Contractor) – EXTENDICARE

We are seeking self-directed, results-oriented nursing leaders with strong communication and critical thinking skills. The Nurse Practitioner will provide innovative clinical leadership to our inter-professional team, supporting knowledge development in geriatric care, and collaborating with physicians, staff, and system partners.

The Nurse Practitioner is an integral part of the leadership team, working within a strong interdisciplinary team. Reporting to the leadership team, including the Administrator, Director of Care, and Medical Director, the NP will provide direct, comprehensive care to Long-Term Care residents with complex needs based on advanced clinical skills. The NP's practice extends beyond regular nursing care, aligning with the full scope of the extended class nurse as per the College of Nurses of Ontario (CNO).

Responsibilities
  1. Deliver primary health care to residents in long-term care, managing acute and chronic conditions, health promotion, and disease/injury prevention.
  2. Provide clinical leadership to LTC team members, including resident admission/discharge, hospital discharge planning, palliative and end-of-life care, pain management, dementia care, and behavioral management.
  3. Conduct comprehensive health assessments, utilizing appropriate tools and techniques, and perform physical examinations, interpreting findings and ordering diagnostic tests.
  4. Collaborate with residents and families to develop person-centered care plans, synthesizing health information to diagnose and assess health risks.
  5. Communicate assessment findings and diagnoses effectively, applying knowledge of pathophysiology, psychopathology, epidemiology, and related fields.
  6. Select and implement appropriate interventions, both pharmacological and non-pharmacological, to maintain or restore resident health.
  7. Promote safe resident care by mitigating harm and addressing risks associated with adverse events.
  8. Liaise with discharge planning services to ensure smooth transitions for hospitalized residents.

Additional duties include collaborating with community health services, participating in quality improvement and research initiatives, and providing education and mentorship to care teams and learners. The NP will dedicate approximately 80% of their time to direct resident care and 20% to research and education. Flexibility to work evenings or weekends and participate in after-hours and on-call coverage is required. Knowledge of RNAO guidelines and MLTC regulations is essential.

Qualifications
  • College of Nurses of Ontario registration in the Extended Class
  • Baccalaureate in Nursing (BScN); Master’s Degree in Nursing (MScN or MN) is an asset
  • Membership with Nurse Practitioners’ Association of Ontario (preferred)
  • Professional liability insurance (with RNAO or other provider)
  • Certification in gerontology (asset)
