Job summary

Pinecrest-Queensway Community Health Centre is seeking a Nurse Practitioner to join the Primary Care and Falls Prevention teams in Ottawa. You will work autonomously, manage a client panel, and provide comprehensive care in a community setting.

The role requires an NP with CNO registration and a minimum of two years of experience, with flexibility for evenings. Collaborative, client-centered care and commitment to health equity are emphasized.

Qualifications

  • Master's degree in Nursing with active CNO registration.
  • Minimum two years NP experience, preferably in primary care/community health.
  • Experience with priority populations is an asset.
  • Knowledge of PS Suite EMR and MS Office is preferred.

Responsibilities

  • Work independently as Most Responsible Provider for a dedicated client panel.
  • Diagnose, treat, and manage acute and chronic conditions across the lifespan.
  • Collaborate with a multidisciplinary team to deliver care.
  • Conduct comprehensive health assessments and develop care plans.
  • Provide education on disease prevention and falls prevention.
  • Maintain documentation in EMR per standards.

Job Summary

The Nurse Practitioner works as part of two unique teams, the Primary Care Team and the Falls Prevention Team. The Primary Care team works as a multi-disciplinary health team to provide primary care services to individuals and families residing in the. The Nurse Practitioner functions as the Primary Care Provider, working autonomously to full scope managing their own panel of clients in accordance with the College of Nurses of Ontario (CNO) standards. The Falls Prevention Team provides community-based care (e.g. home visits) in collaboration with a multi-disciplinary health care team, including physio, occupational therapist and nurse. The Nurse practitioner will work in the provision of individual client medical assessments as it relates to falls and fall prevention.

Job Specific Responsibilities for Primary Care:
  • The NP works independently, serving as the Most Responsible Provider for a dedicated panel of clients, with access to physician consultation as appropriate.
  • Diagnose, treat, and manage acute and chronic conditions across the lifespan, including for older adults in a community / home setting.
  • Collaborate with physicians, registered nurses, allied health professionals, and social services staff to deliver team-based care and recommendations.
  • Conduct comprehensive health assessments, develop and review care plans, and provide direct clinical services.
  • Order and interpret diagnostic investigations, prescribe medications, and coordinate referrals to specialists and community resources.
  • Educate clients on disease prevention, health promotion, and chronic disease management. Participates in community education sessions promoting falls prevention.
  • Maintain accurate and timely documentation in the electronic medical record in accordance with professional and organizational standards.
  • Participate in team meetings, case conferences, quality improvement, and continuing professional development activities.
  • Support clients experiencing health inequities linked to the social determinants of health, such as income, housing, and employment.
Qualifications
  • Master's degree in Nursing and current registration in good standing with the College of Nurses of Ontario (CNO) in the Extended Class (NP-PHC or NP-Adult).
  • Minimum of two (2) years of experience practicing as a Nurse Practitioner, ideally in a primary care or community health setting.
  • Demonstrated experience or strong interest in working with one or more of PQCHC's priority populations: newcomers and refugees, older adults, 2SLGBTQ+ and gender-diverse clients, and people experiencing health inequities linked to the social determinants of health, such as income, housing, and employment.
  • Experience in a community setting desirable.
  • Demonstrated commitment to and knowledge of community-based health care.
  • Technology proficiency an asset – PS Suite EMR, Microsoft Office Suite.
  • Flexible hours, including evenings.

  • Excellent communication and interpersonal skills.
  • Linguistic profile: Bilingual English/French is an asset.
