Job description

This nurturing process is ideal for building therapeutic relationships, promoting a strong continuity of care, and giving healthcare practitioners full control over their own schedules. Spark Lifecare is proud to be an inclusive and equity-focused organization.

Spark is seeking clinically strong, driven, and business-minded Nurse Practitioners for providing services across Ontario. Our clients are lovely individuals seeking nurse practitioner support in the form of in-home assessments and follow-ups. After experiences such as acute illness/injury, trauma, complex medical diagnoses, or surgeries, clients are looking for support to help guide their recovery. This may include physical exams, provision of diagnoses, and requisitions for diagnostics as appropriate in an environment in which they may not be readily able to access these supports outside of the hospital setting.

You would be the ideal candidate to work with us if you are passionate about helping people, are reliable, love working in a team, are registered as a Nurse Practitioner (NP) with the College of Nurses of Ontario, and have experience working with individuals in the community.

We are excited to work with you because:

  1. You have experience working in a community-based setting.
  2. You have experience in advising patients and their families on health care needs including health promotion, disease, illness and accident prevention.
  3. You are an advocate for client-centered care.
  4. You can collaborate with members of an interdisciplinary health team such as therapists, caregivers, case managers to plan, implement, coordinate and evaluate patient care in consultation with patients and their families.

Hours:
Part-Time: Flexible scheduling options are available.

Rate of Pay:
70% of billable rate.

Requirements:

  • Cover letter outlining why you would be a great match for Spark. Tell us who you are!
  • Valid First Aid and CPR Standard level C certification.
  • Smartphone with a data plan.
  • A Clear Criminal Record & Vulnerable Sector Check (within the last 6 months).

Assets:

  • Experience working in a multi-disciplinary team.
  • Drivers License G class with access to a vehicle.

Why Spark?

  • Competitive compensation.
  • Strong administrative support.
  • Flexible caseload and schedule.
  • Access and coverage for management softwares, online booking systems, and digital charting.
  • Access to mentorship and resources to support your practice.
