For more than 50 years, Grove Park has been a trusted provider of a full continuum of services, including long-term care, a boutique retirement residence, and an Adult Day Program, all focused on supporting dignity, independence, and quality of life for every individual we serve.

Currently undergoing an exciting multi-year redevelopment, Grove Park Home is temporarily operating as a 113-bed long-term care home, with plans to expand to 145 beds upon completion. Job Summary:

Reporting to the Administrator and Director of Care, the Nurse Practitioner (NP) provides advanced clinical leadership and comprehensive primary care to residents at Grove Park Home, supporting the delivery of high‑quality, person‑centred services within the long‑term care environment. The NP is responsible for independent clinical assessment, diagnosis, treatment, and chronic disease management, working collaboratively with the Medical Director, attending physicians, nursing leadership, and the interdisciplinary team to optimize resident outcomes, safety, and quality of life.

The Nurse Practitioner plays an integral role in advancing clinical quality improvement, evidence‑based practice, and interdisciplinary care planning. This includes conducting comprehensive health assessments, medication reviews, diagnostic follow‑up, wound and skin management, and supporting pain, palliative, and end‑of‑life care. The NP also provides consultation, education, and mentorship to nursing staff and care teams, contributing to continuous improvement in professional practice, regulatory compliance, and resident‑centred care delivery.

Duties &Responsibilities:

Advanced Clinical Practice & Resident Care Leadership

  • Provide advanced clinical assessment, diagnosis, and comprehensive primary and chronic disease management for long‑term care residents, practicing to full Nurse Practitioner scope.
  • Lead person‑centred health assessments, medication management, diagnostic investigation and follow‑up, and evidence‑informed therapeutic interventions to support optimal resident health outcomes, comfort, and quality of life, including pain control, wound management, and palliative and end‑of‑life care.

Physician Partnership & Interdisciplinary Care Integration

  • Serve as a key clinical partner to the Medical Director, attending physicians, and nursing leadership, ensuring coordinated, responsive, and proactive resident care.
  • Collaborate closely with interdisciplinary teams, residents, and families to develop, implement, and continuously evaluate individualized plans of care.
  • Facilitate timely specialist referrals, clinical consultations, and continuity of care while supporting complex decision‑making and risk mitigation.

Clinical Leadership, Mentorship & Professional Practice Excellence

  • Provide visible clinical leadership across the Home, offering consultation, coaching, and mentorship to RNs, RPNs, PSWs, and interdisciplinary team members to strengthen clinical decision‑making, professional practice, and resident safety.
  • Support staff education initiatives, contribute to competency development, and guide teams in managing complex or evolving clinical situations in alignment with legislative requirements and best practices in long‑term care.

Quality Improvement, Outcomes & Evidence‑Based Innovation

  • Champion continuous clinical quality improvement by monitoring resident outcomes, identifying trends, and supporting implementation of research‑informed practices that enhance care delivery and resident experience.
  • Participate in organizational initiatives focused on clinical excellence, inspection readiness, and practice improvement, contributing to a culture of accountability, learning, and innovation across the Home.

Documentation, Compliance & Professional Accountability

  • Ensure accurate, timely, and defensible clinical documentation reflecting assessments, diagnoses, care plans, interventions, and resident responses.
  • Maintain full compliance with College of Nurses of Ontario standards, applicable legislation, and Grove Park Home policies while fulfilling all professional responsibilities consistent with the Nurse Practitioner role.
Skills & Qualifications:

Education & Registration

  • Current registration in good standing as a Nurse Practitioner (RN-EC) with the College of Nurses of Ontario.
  • Current CPR certification (Heart Save Level or equivalent).
  • Completion of all professional licensing requirements necessary to practice within Ontario long‑term care.

Clinical Experience

  • A minimum of one (1) year of experience practicing as a Nurse Practitioner is preferred.
  • Experience in a long‑term care, geriatric, or complex continuing care environment considered a strong asset.
  • Demonstrated experience providing primary care, chronic disease management, and comprehensive clinical assessment within an interdisciplinary healthcare setting.
  • Advanced wound care experience considered an asset.

Knowledge of Long‑Term Care & Regulatory Environment

  • Strong knowledge of primary health care issues affecting older adults and medically complex residents.
  • Understanding of applicable legislation, regulations, and professional standards governing Ontario long‑term care practice.
  • Familiarity with medication management, quality improvement processes, and evidence‑based clinical practice in a residential care setting.
Compensation:
  • The position offers an hourly rate of $80.00, based on a regular schedule of 3 days per week, excluding Mondays.
