Perennial delivers integrated primary care and wellness services directly within retirement residences, combining on‑site and virtual care to meet seniors where they are. By proactively anticipating the needs of older adults, we improve quality of life, reduce unnecessary hospital visits, and ease the strain on the public healthcare system.

Our interdisciplinary team—physicians, nurse practitioners, registered nurses, physiotherapists, occupational therapists, dietitians, and massage therapists—works together with one clear mission: to keep residents healthy, active, and comfortable in their own homes.

About The Role

We are seeking a part‑time Nurse Practitioner (NP) to join our team in London, Ontario, with a start date between December 2025 and January 2026. This role requires a commitment of approximately 1–2 days per week, with flexibility around specific days and hours to best fit your schedule. A consistent weekly schedule will be established, so you will have set clinic days each week. This ensures continuity of care for residents and allows you to plan your professional and personal commitments with ease.

As a key member of our clinical team, you will provide comprehensive primary care services to senior patients in both in‑person and collaborative settings.

  • Conducting routine check‑ups and managing chronic and acute conditions
  • Ordering and interpreting diagnostic tests
  • Prescribing medications and developing care plans
  • Addressing day‑to‑day medical concerns raised by residents and families
  • Collaborating closely with our interprofessional healthcare team to ensure seniors receive the highest standard of care
What You’ll Do
  • Provide Primary & Urgent Care: Act as the primary healthcare provider for seniors, delivering timely assessments, diagnoses, and treatments. Offer on‑site care at the retirement residence in Hamilton and provide follow‑ups when needed.
  • Conduct Patient Consultations: Carry out thorough medical consultations and assessments, tailoring care plans to each patient’s individual needs.
  • Develop & Manage Care Plans: Create and implement personalized treatment plans, including preventative measures, chronic disease management, and ongoing monitoring.
  • Prescribe & Administer Treatments: Order diagnostic tests, prescribe medications, and administer treatments in alignment with best practice and patient safety standards.
  • Educate Patients & Families: Provide education on disease prevention, health management, medications, and lifestyle strategies to support overall wellness.
  • Promote Preventative Health: Lead proactive care initiatives such as immunizations, screenings, and risk assessments to optimize long‑term health outcomes.
  • Maintain Accurate Documentation: Ensure timely and precise charting within electronic medical records (EMRs), maintaining compliance with privacy and regulatory requirements.
  • Collaborate Across Disciplines: Work closely with physicians, NPs, and allied health professionals to ensure seamless, coordinated care for residents.
  • Support Quality & Best Practices: Contribute to continuous improvement initiatives and remain current with evidence‑based guidelines in geriatric and primary care.
Qualifications
  • Clinical Experience: Minimum of 2+ years of hands‑on nursing practice, ideally in primary care, urgent care, geriatrics, or a related setting.
  • Education: Master of Nursing (MN) or equivalent graduate degree with a clinical focus.
  • NP Certification: Successful completion of a Nurse Practitioner program (MN or post‑master’s certificate).
  • Licensure: Current registration in good standing with the College of Nurses of Ontario in the RN (Extended Class).
  • Clinical Skills: Strong ability to assess, diagnose, and manage acute and chronic health conditions.
  • Communication: Exceptional communication and interpersonal skills, with proven ability to support and educate older adults, families, and caregivers.
  • Teamwork: Comfortable working collaboratively within a multidisciplinary healthcare team.
  • Passion for Seniors’ Health: Experience or demonstrated interest in geriatric care, preventative health, and wellness promotion.
