Qualifications

Active licensure to practice in the province of employment.
Certified Nurse Practitioner credential required.
ANCC/AANP certification or equivalent preferred.
Interest in Physiatry/Physical Medicine and Rehabilitation.
Orthopedic experience is a plus; Physiatry experience is a plus.

Responsibilities

Collaborate with therapists, nurses and other healthcare professionals to develop comprehensive rehabilitation plans.
Conduct patient assessments including medical history and examinations to set rehab goals.
Monitor progress and adjust treatment plans to maximize function and independence.
Participate in multidisciplinary team meetings and continuing education activities.
Maintain accurate documentation of evaluations, plans, progress notes and discharge summaries in EMR systems.
Educate patients and families about the rehabilitation process and post-discharge care.

Essential Duties & Responsibilities:

Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions.
Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence.
Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems.
Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care.
