Job summary

Chestnut Health in Canada is seeking a Physician Assistant to join our physiatry team as part of a leading multidisciplinary rehabilitation program. You will collaborate with therapists, nurses, and physicians to develop and implement comprehensive treatment plans aimed at restoring function and independence.

The role emphasizes patient-centered care, thorough assessments, progress monitoring, and education for patients and families.

Qualifications

  • Licensed to practice as a Physician Assistant
  • Eligible to work in the United States
  • NCCPA certification preferred
  • DEA license preferred
  • Controlled substance license preferred
  • Orthopedic experience is a plus
  • Physiatry experience is a plus
  • Skilled nursing experience is a plus

Responsibilities

  • Collaborate with therapists, nurses, and other professionals to develop comprehensive treatment plans
  • Conduct patient assessments including histories and examinations
  • Monitor progress and adjust plans to improve outcomes
  • Participate in multidisciplinary meetings and education activities
  • Document evaluations, plans, progress, and discharge summaries in EMR
  • Educate patients and families about rehabilitation and post-discharge care

Essential Duties & Responsibilities:
  • Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
  • Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions.
  • Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence.
  • Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
  • Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems.
  • Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care.
Qualifications:
  • Actively licensed to practice as a Physician Assistant in the state of employment
  • Eligible to work in the United States
  • Current Controlled Substance License preferred upon start date
  • Current DEA License preferred upon start date
  • NCCPA Certificate preferred
  • Orthopedic experience is nice to have
  • Physiatry experience is nice to have
  • Skilled nursing experience is nice to have
