Sioux Lookout First Nations Health Authority is seeking a Nurse IV - NP to provide primary health care in the Sioux Lookout catchment, serving 33 First Nations communities. You will assess, diagnose, and support patients with acute illnesses and chronic disease management, collaborating with an interdisciplinary team to deliver holistic care across all ages.

The role emphasizes community health promotion, disease prevention, and case management within a high-need region.

Qualifications

• Current registration as a Registered Nurse in the extended class.
• Must hold liability insurance.
• Minimum 3 years of Nurse Practitioner clinical experience.
• Experience with First Nations communities preferred.
• Willingness to learn about communicable diseases in the served population.
• Ability to work in a multi-disciplinary team.
• Strong relationship-building and communication skills.

Description

Under the Northern Clinic and within the scope of practice of a Nurse Practitioner (NP), the Nurse IV - NP will be responsible for providing primary health care to clients within the Sioux Lookout area. The NP assesses and diagnoses client health for acute illnesses and supports clients to manage chronic illnesses. The NP works with the client and the community to promote healthy lifestyles and disease prevention. The NP collaborates and consults with members of an interdisciplinary team to address issues experienced by clients and within the community. The NP will provide holistic and integrated care to all ages within the Sioux Lookout catchment area, including the 33 First Nations communities.

The NP will:

The Nurse IV - NP will work out of the Northern Clinic department and will be directly accountable to the Manager - Northern Clinic.

Salary:$116,114 - $169,608 per annum.

As an approved employer under Ontario Health's Community Commitment Program for Nurses (CCPN), eligible Nurse Practitioners may qualify for up to $45,000 in financial incentives, including the Community Commitment Grant, Northern Top-Up Grant, and Northern Relocation Grant.

The CCPN is designed to attract nurses to high-need communities across Ontario by providing financial support in exchange for a two-year full-time commitment with an eligible employer. Eligibility and funding are subject to Ontario Health program requirements and approval.

• Current registration as a Registered Nurse in the extended class, with the College of Nurses of Ontario.

• Awareness of and sensitivity to the health issues of a diverse community and experience working with First Nations communities.


• Demonstrated ability to work effectively in a multi-disciplinary

• Ability to communicate in one of the First Nations dialects of the Sioux Lookout region is an asset
