Job summary

SE Health in Scarborough is hiring full-time and part-time days and evenings nurses to deliver compassionate community care in the Scarborough area. The role offers a supportive team, flexible schedules, paid training and competitive pay.

Responsibilities include client assessments, care planning and transitions, collaborating with the care team, and ensuring care meets evidence-informed standards. Ontario RN registration and CPR are required, along with a valid driver's license and vehicle.

Qualifications

• Current registration with the College of Nurses of Ontario is required.
• CNA specialty certification is an asset to the role.
• A valid CPR certificate is required.
• A valid driver’s license and vehicle are required as travel is involved.

Responsibilities

• Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence and client needs.
• Serve as primary provider for a client caseload and collaborate with the care team and support functions.
• Support clients through transitions by connecting with community support services.

Education

Current registration with the College of Nurses of Ontario

CNA specialty certification (asset)

CPR certificate
