Job description

The NMCCoordinator, as a key member of the Lead Health Provider team responsible forsupporting community members with health system navigation and coordination ofsupports addressing holistic care needs, while promoting independence andincreasing client ability to self-manage care.

We are seeking a compassionate andorganized Neighbourhood Model of Care NMC Coordinator to lead and coordinatecare and community support for seniors living in an designated building. Thisrole supports early intervention, risk mitigation, and service navigation,acting as a consistent, trusted point of contact for clients, families, andservice providers.

KeyResponsibilities:

  • Conduct health assessments todetermine identified community member needs.

· Provideinformation and support referrals to external community resources.

· Onboardcommunity members who qualify for NMC personal support services and monitortheir changing health needs.

· PromoteNMC personal support services and link with community services to presentphysical, mental and financial literacy programs encourage community members totake an active role in their health and wellness needs

  • Provide tailoredperson-centered support for community members including systemsnavigation, care coordination, communication, and collaboration with thecommunity member’s circle of care to ensure best outcomes for wellness.
  • Highly organized and excels atkeeping client files and information accurate, complete, and confidential.
  • Research, identify, implement, and promote wellnessinitiatives that support a healthy, resilient, and engaged community.
  • Participate in monthly meetings with the NENeighbourhood Model of Care Team.
  • Participate in a monthly structured meeting with OntarioHealth at Home and the Housing Provider to review and develop communitywellness events and collaborate in ethical problem-solving involvingsituations of concern.
  • Contributeto the planning, monitoring, review and evaluating of Neighbourhood Modelof Care programs/services for continuous quality improvement.

Qualifications:

  • Diploma or degree in socialservices, community health, gerontology, or related field
  • 2+ years’ experience workingwith older adults or vulnerable populations in a community setting
  • Knowledge of the socialdeterminants of health, seniors’ wellness, and community care systems
  • Excellent communication,interpersonal, and organizational skills
  • Ability to work independentlyand respond effectively in crisis situations
  • Proficiency with electronichealth records and mobile documentation tools, good working of knowledgeof Excel, Word and Outlook
  • Valid driver’s license and Useof own personal Vehicle with two (2) million dollar liability Insurance.

Assets:

  • Experience with seniors livingin congregate housing or independent living settings
  • Knowledge of local health andsocial service networks
  • Multilingual abilities andcultural competency are strong assets
