Extendicare is seeking a Part-time Chaplain/Spiritual Care Practitioner to support residents, families and staff in a multi-faith environment. The role focuses on spiritual care, counselling, and coordinating pastoral services within the facility to enrich residents' quality of life.

The position emphasizes leading religious activities, providing cultural care, and contributing to bereavement and care planning documentation.

Qualifications

• Ability to lead prayer groups, study groups, church services and celebrations for all faiths.
• Provide spiritual care and supportive counselling to residents, families and staff.
• Assess spiritual and religious needs in a multi-faith environment and coordinate with the care team.

Responsibilities

• Assess spiritual and religious care needs and provide direct spiritual support and religious care to residents, families and staff.
• Provide spiritual care and supportive counselling and refer to other team members or partners as needed.
• Coordinate, orientate and evaluate pastoral service providers for the facility.
• Lead prayer groups, study groups, church services and faith celebrations.
• Participate in quarterly memorial or bereavement functions.
• Document care planning and progress notes.
