Fraser Health Authority is hiring Casual Social Workers (MSW) to join the team at Surrey Memorial Hospital and Jim Pattison Outpatient Care & Surgery Centre in Surrey, BC. The role involves comprehensive social work and psychosocial assessments, counseling, and collaboration with a multidisciplinary team to support patient care planning and discharge planning.

Qualifications include a Master’s degree in Social Work, current BC College of Social Workers registration, and a year of acute care

Qualifications

• Master’s degree in Social Work, plus one year recent acute care clinical experience.
• Current full registration with the BC College of Social Workers.
• Applicants with a Bachelor of Social Work may be considered under equivalency.

Responsabilités

• Conduct comprehensive social work and psychosocial assessments of patients and families.
• Provide grief and bereavement counseling and coordinate care with the interdisciplinary team.
• Educate and consult with patients, families and healthcare professionals on high-risk factors and recovery.
• Maintain patient records and prepare progress notes and reports.
• Lead and participate in care planning, discharge planning and community service coordination.
