Job summary

Fraser Health Authority is hiring Casual Social Workers for Long Term Care in Surrey, BC. You will conduct psycho-social assessments, provide clinical counseling, and support families and residents facing chronic illness and transition challenges.

The role collaborates with an interdisciplinary team to plan care and connect clients to community resources. Reporting to the Professional Practice Leader, you’ll contribute to care planning, participate in in services, and help maintain documentation

Qualifications

• Bachelor’s degree in Social Work from an accredited School of Social Work.
• One year of recent related experience or an equivalent combination of education, training and experience.
• Current full registration with the BC College of Social Workers.

Responsibilities

• Conduct psycho-social assessments of clients/residents and families.
• Provide clinical counseling for adjustment to chronic illness, disability, relocation, bereavement, and caregiver support.
• Coordinate admissions with interdisciplinary staff and provide admission orientation to clients/residents and families.
• Contribute to client/resident care plans by assessing emotional, social and physical needs.
• Facilitate interdisciplinary team conferences and organize meetings with providers, clients/residents and families.
• Provide community resource information and facilitates referrals to services in the community.
• Maintain record keeping and reporting including charting on medical charts and workload statistics.
