Providence Health Care, based in Vancouver, seeks a Social Worker to provide therapeutic counselling and psychosocial assessments within an interdisciplinary team. You will coordinate services, maintain records, and contribute to discharge planning under supervision.

Qualifikationen

• Master of Social Work from an accredited university.
• One year of recent related experience in a designated program area, or an equivalent combination of education, training and experience.
• Current full registration with the British Columbia College of Social Workers.
• Bachelor of Social Work degree from an accredited School of Social Work, with an equivalent combination of education, training and experience may be considered.
• Current full registration with the British Columbia College of Social Workers.

Aufgaben

• Provides therapeutic clinical counselling services to patients/residents, families and groups in designated program area, coordinating services with interdisciplinary teams.
• Conducts comprehensive social work and psychosocial assessments.
• Maintains patient/resident records and documentation per standards and policies.
• Contributes to discharge planning as part of the interdisciplinary care team.
• Provides teaching, supervision and training to students and participates in group and committee activities.
