Job summary

Bayshore HealthCare is seeking a Manager of Clinical Practice to lead our Bundled Care Program, supporting clients and caregivers through high-quality, community-based care.

In this leadership role, you’ll collaborate with interdisciplinary teams, mentor frontline staff, and ensure care delivery meets the highest standards of excellence and compassion. This position is based in Ontario and requires flexibility for evenings/weekends.

Qualifications

  • Minimum 5 years in healthcare.
  • Registered Practical Nurse (Ontario) with CPR certification.
  • Strong understanding of community nursing and discharge planning.
  • Proven ability to mentor and lead teams with compassion and consistency.
  • Valid driver’s license and reliable vehicle.
  • Familiarity with RAI tools is an asset.

Responsibilities

  • Lead the delivery of holistic care by collaborating with nursing, personal support workers, allied health professionals, and social workers.
  • Partner with hospitals and community organizations to ensure safe and effective patient transitions.
  • Provide mentorship and supervision to staff within your program.
  • Ensure all services meet provincial standards and internal policies.
  • Design and implement individualized care plans in collaboration with hospital partners.

Bayshore’s Integrated Care Solutions is seeking a Manager of Clinical Practice to lead our Bundled Care Program, supporting clients and caregivers through high-quality, community-based care. In this leadership role, you’ll collaborate with interdisciplinary teams, mentor frontline staff, and ensure care delivery meets the highest standards of excellence, compassion, and reliability.

What You’ll Do
  • Lead the delivery of holistic care by collaborating with nursing, personal support workers, allied health professionals, and social workers.
  • Partner with hospitals, Home and Community Care Support Services (HCCSS), and other community organizations to ensure safe and effective patient transitions.
  • Provide mentorship, supervision, and performance management to personal support workers and nurses within your program.
  • Ensure all services meet federal and provincial standards, as well as internal policies and best practices.
  • Conduct case reviews and monitor client outcomes to ensure care goals are being met efficiently and effectively.
  • Design and implement individualized care plans in collaboration with hospital partners to support safe discharges and continuity of care.
  • Address patient and family concerns with professionalism and empathy, ensuring timely follow-up and resolution.
  • Support staff development through home visits focused on training, performance evaluation, and skill-building.
  • Analyze intake processes, priority coding, and service refusals, and implement strategies to improve access and efficiency.
  • Oversee the completion of client assessments and ensure documentation is thorough and accurate.
  • Collaborate with the Clinical Manager to track program metrics and communicate outcomes to the data management team.
  • Escalate care or service-level concerns to the Clinical Manager for further review and resolution.
  • Foster strong internal and external relationships through clear, compassionate communication.
  • Participate in case conferences, team meetings, and ongoing professional development activities.
  • Be available for flexible scheduling, including evenings and weekends, and participate in the on‑call rotation as needed.
What You Bring
  • Minimum 5 years in healthcare, preferably in home care and hospital settings.
  • Registered Practical Nurse (Ontario), graduate of an accredited program, current CPR certification.
  • Strong understanding of community nursing, service delivery, discharge planning, and Ontario’s health system.
  • Proven ability to mentor and lead teams with compassion and consistency.
  • Familiarity with RAI tools is an asset.
  • Valid driver’s license and reliable vehicle.
