Job Summary:
Provides nursing care to patients under the supervision of a Registered Nurse or physician/advanced practice provider. Utilizes observational skills to monitor nursing interventions and assists medical staff to ensure optimal care within established policies, procedures, and standards. Performs duties in accordance with the legal scope of practice and organizational standards, demonstrating alignment with the Mission, Vision, and Strategic Plan of the organization. Maintains quality and effectiveness in work habits and clinical practice, treating coworkers, patients, families, and all members of the healthcare team with dignity and respect.
Essential Responsibilities:
- Exhibits excellent customer relations with patients, visitors, providers, and co-workers; shows courtesy, compassion, and respect; adheres to department service standards.
- Provides therapeutic patient care within the scope of practice as established by physicians/advanced practice providers and registered nurses.
- Transcribes physician/advanced practice provider orders following policy.
- Communicates with registered nurses to modify the care plan; assists in developing nursing care plans.
- Administers medications accurately according to provider orders and standing orders within scope of practice.
- Performs treatments such as urinary catheterizations, IV placements, irrigations, suctioning, throat irrigations, enemas, and uses equipment as required, following infection control policies.
- Adheres to high-level disinfectant, point of use, peel packs policies, and procedures.
- Performs patient care functions like taking vital signs, wound care, instructing patients on self-administered treatments, collecting specimens, and ensuring proper culture collection.
- Assists medical staff with examinations and treatments by passing instruments, supporting and restraining patients, draping, and other tasks as directed.
- Observes and documents patient conditions, signs, symptoms, and reactions; initiates emergency procedures if necessary; reports significant findings promptly.
- May start and maintain IV therapy with certifications, notifying appropriate staff of issues; may discontinue IVs except intra-caths and central lines.
- May perform venipuncture/blood withdrawal with certifications.
- Collects and processes specimens per policies and protocols.
- Provides non-complicated patient education following the care plan; reports learning needs and discharge barriers.
- Ensures cleanliness of work areas and proper functioning of equipment to maintain a safe environment.
- Uses effective verbal and non-verbal communication skills.
- Performs duties in other departments or modules as assigned to maintain standards of care and safety.
- Handles departmental workflows including patient outreach, closing care gaps, and nurse visits.
- Participates in staff meetings and in-service training programs.
- Assists with ordering and stocking supplies and equipment.
- Accepts and documents verbal and telephone orders from providers.
- Supports patient communication and message management, including emails and call center messages.
- Administers oxygen therapy, suctioning, sputum induction, and documents care appropriately.
- Performs other duties as assigned, including orientation of new staff and discharge coordination.
- Operates within license and scope of practice, promoting excellent customer service and compliance with policies and laws.
- Utilizes skills assessments to ensure proficiency with age-specific patient needs.

- Minimum of twelve (12) months or greater LVN experience.
