Job Summary

Provides nursing care to patients under the supervision of a Registered Nurse or physician/advanced practice provider. Utilizes observational skills to monitor nursing interventions and assist medical staff to ensure optimal care within established policies, procedures, and standards. Performs nursing duties in accordance with legal scope of practice, standards of care, policies, and procedures. Demonstrates commitment to the organization’s Mission, Vision, and Strategic Plan, maintaining quality and effectiveness in work habits and clinical practice, and treating coworkers, patients, families, and all members of the healthcare team with dignity and respect.

Essential Responsibilities
  1. Exhibit excellent customer relations with patients, visitors, providers, and co-workers; show courtesy, compassion, and respect; adhere to department service standards.
  2. Provide therapeutic patient care as established by physicians/advanced practice providers and/or registered nurses within scope of practice.
  3. Transcribe physician/advanced practice provider orders and follow policy guidelines.
  4. Communicate directly with registered nurses to modify the care plan; assist in developing nursing care plans.
  5. Administer medications accurately following provider orders or standing orders within scope of practice.
  6. Perform treatments such as urinary catheterizations, IV placements, bladder irrigations, suctioning, throat irrigations, enemas, and use of equipment, adhering to infection control policies.
  7. Maintain infection prevention standards, including disinfectant and peel pack policies.
  8. Perform patient care functions like vital signs, wound care, patient education, specimen collection, and culture accuracy.
  9. Assist physicians/advanced practice providers with examinations and treatments, supporting patient comfort and safety.
  10. Observe and document patient conditions, identify abnormal findings, initiate emergency procedures if needed, and report to the appropriate staff.
  11. Volunteer to start and maintain IV therapy and perform venipuncture with certifications; notify staff of problems.
  12. Process specimens per protocols.
  13. Provide patient education following care plans, reporting learning needs and discharge barriers.
  14. Ensure cleanliness and proper functioning of care areas and equipment, maintaining a safe environment.
  15. Use effective communication skills.
  16. Perform departmental workflows, including patient outreach and care gap closures.
  17. Participate in staff meetings and training.
  18. Assist with supply management and order processing.
  19. Handle verbal and telephone orders from providers.
  20. Manage patient communication channels and messages.
  21. Administer oxygen therapy, suctioning, and sputum induction.
  22. Document care activities in Health Connect or relevant systems.
  23. Perform additional duties such as orientation of new staff and discharge coordination.
  24. Perform all functions within licensure and scope of practice.
  25. Promote customer service excellence and organizational compliance, including confidentiality and legal adherence.
  26. Utilize skills assessments to ensure proficiency in caring for specific patient age groups.
Experience
  • Minimum of 12 months LVN direct patient care experience within the last three years, including medical office, acute care, home care, or skilled nursing facility.
Education
  • High School Diploma or GED.
