Job summary

Alberta Health Services in Red Deer is hiring a Licensed Practical Nurse for Intensive Home Care. You will work with clients and families to deliver safe, patient-centred community care under the area manager and interprofessional team.

You will support early discharge, prevent hospital admissions when possible, and may cover other assignments based on workload. AHS emphasizes collaboration with stakeholders and ongoing quality care.

Qualifications

• Completion of an accredited practical nurse education program.
• Active or eligible for registration and a practice permit as a Licensed Practical Nurse with the College of LPNs and HCAs of Alberta (CLHA).
• Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

Responsibilities

• Interact with the team and clients/families to provide quality care in the community.
• Facilitate early hospital discharge and prevent hospital admissions when appropriate.
• Coordinate referrals to stakeholders and provide direct care as needed.
