• Assess patient/resident/family needs across physical, psychological, socio-cultural, and spiritual areas.
• Provide a supportive environment and facilitate care decisions with patient/family.
• Contribute to individualized care plans and multidisciplinary meetings.
• Deliver nursing care per policies and FHA standards; document observations.
• Handle admissions, discharges, and transfers per policies.
• Maintain accurate care documentation to ensure safety and comfort.
