Job summary

ParaMed is seeking a Clinical Supervisor in Ontario to oversee PSW/HSW teams delivering home health care. You will lead clinically, coordinate care, and ensure compliance with standards while supporting patients and families in the community.

The role requires RN or RPN registration in Ontario, 5+ years of nursing leadership, strong assessment and documentation skills, and a valid driver’s license with reliable transportation. This replacement position offers comprehensive benefits.

Qualifications

  • Current RN or RPN registration with the College of Nurses of Ontario and entitled to practice without restrictions.
  • Minimum of 5 years’ experience as a practicing RN or RNPrn.
  • Comprehensive patient health assessment skills are required.
  • Understanding of community nursing practice principles.
  • Proven experience leading and managing a team.
  • Knowledge of clinical documentation standards and practice guidelines.
  • Excellent organizational, interpersonal, and teaching skills.
  • Commitment to professional development in home health care.
  • Effective critical thinking and problem-solving skills.
  • Must possess a valid driver’s license and access to reliable transportation.
  • Vulnerable Sector Screening (VSS) clearance is required.
  • CNA Certification as a Community Health Nurse (CCHN(C)) is considered an asset.

Responsibilities

  • Supervise and support PSW’s and HSW’s to ensure practices comply with legislated standards of practice as well as company policies and procedures.
  • Respond to and advise/resolve concerns from community staff, patients/families, and other team members.
  • Provide ongoing leadership through regular huddles, face to face meetings and one-on-ones.
  • Initiate patient safety incident investigations and documents to ensure timelines and action plans are met.
  • Work collaboratively with nursing team members to support clinical documentation and submission requirements according to specific funder reporting guidelines.
  • Engage patients in problem solving using a collaborative approach, offering health teaching, building rapport and trust to identify patient health care needs, strengths, capacities, and goals.
  • Engage staff in problem solving, offering education, coaching, and mentoring in resolution of problems.
  • Maintain the knowledge, skills, and attitudes to provide safe, efficient, effective service in all areas of practice by keeping abreast of changes in practice setting that affect scope of practice.
