Right at Home Canada Fraser Valley is seeking an experienced LPN to oversee care planning and day-to-day office operations in Abbotsford. The Care Planner will coach caregivers, coordinate client care, and ensure timely follow-up with families while visiting client homes.

The role is in-person, based in Abbotsford, with regular visits across the Fraser Valley. You will lead a small home-care team, maintain client records, and support business growth through strong client and referral

Qualifikationen

• Current practising LPN registration with BCCNM, in good standing.
• Home care, community care, dementia care or care coordination experience preferred.
• Supervisory experience, including coaching staff and addressing performance concerns.

Aufgaben

• Oversee care planning and day-to-day office operations.
• Match caregivers to clients based on skills, availability and suitability.
• Provide orientation and coaching in the client's home when needed.
• Complete supervisory visits and follow up with clients, families and caregivers.
• Maintain accurate client records and communicate changes to the team.
• Support staffing decisions and caregiver continuity to grow hours.
