Zusammenfassung

Fraser Health Authority is seeking an experienced Licensed Practical Nurse to support Indigenous Care-Giver Home Care in British Columbia. The role focuses on culturally safe home-based nursing tasks and coordinating with the Indigenous Health team to ensure client-centred care.

The LPN will mentor caregivers, assess client needs, participate in care planning, and administer medications within scope. A BC driver's license and vehicle are required.

Qualifikationen

• Graduation from an approved school in Practical Nursing.
• One year recent, related clinical and/or mentoring experience in a community health setting, or an equivalent combination of education, training and experience.
• Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nursing Professionals (BCCNP).
• Valid BC Driver's License and access to a personal vehicle for business-related purposes.

Aufgaben

• Visits assigned care-givers in the clients home to assess skills and abilities; maintains records and documentation.
• Supports designated care-givers by determining training and orientation requirements.
• Assesses and evaluates client care needs in accordance with standards; guides care-givers and supports the clinical team.
• Contributes to development of the client's care plan with Indigenous Health Clinical Resource Nurse; communicates with home health providers and allied services.
• Contributes to development and revision of care standards and policies; supports quality improvements.
• Performs nursing care and procedures; evaluates outcomes; administers medications at home per policy.
• Responds to inquiries by telephone and in person; provides information about programs and policies.
• Assists in selecting and identifying appropriate care-givers with families and community supports.
