Qualifications

  • Infirmier(ère) pratique inscrit(e) et actif(ve) par la CLPNA.
  • Validité du permis de conduire et accès à un véhicule.
  • Expérience en soins communautaires préférée.

Job description

Community Nursing Role - Licensed Practical Nurse (LPN)

In this community nursing role, you as a Licensed Practical Nurse (LPN) will have scheduled visits within the community to care for patients in various settings with the support of RNs for more complex patients. You will work with patients of all ages with differing care needs and play a key role in helping patients remain comfortable in their homes for as long as possible. What you’ll do:

  • Participate in scheduled community visits to address patient needs within your scope of practice
  • Provide ethical and safe nursing care
  • Use assessment tools and techniques to evaluate patient needs
  • Implement and monitor care plans, adjusting as necessary
  • Build rapport and trust with patients through a collaborative approach
  • Maintain and update professional patient records

What you bring:

  • Registration with the CLPNA as an active Licensed Practical Nurse (LPN)
  • Valid driver’s license and access to a vehicle
  • Experience in community care is preferred
  • Reliability and flexibility
  • Willingness to travel within a designated area
  • Experience working with vulnerable populations
  • Passion for community service
  • On-call availability, including after hours, for patient safety and care needs
