Reporting to the designated manager within Home and Community Care, the Long Term Care Case Manager, Nurse, works as part of an inter-professional healthcare team. Responsibilities include assessing, planning, organizing, reviewing, and evaluating the care needs of clients and their caregivers/families requiring community-based services. The role fosters continuity of care and promotes partnerships by liaising with primary care physicians, long-term care homes, hospitals, and other organizations. The position may operate in various settings such as Long-Term Care, assisted living, acute care, and community.
Registration with BC College of Nurses and Midwives as a practicing RN. Three (3) years recent related experience in gerontology, including two years in community health; or an equivalent combination of education, training, and experience. Valid BC Driver's license.
