The Thunder Bay Indigenous Friendship Centre is seeking a Life Long Care Support Worker to develop and provide culturally appropriate community support services for urban Indigenous seniors, frail, physically disabled, and chronically ill individuals, promoting independent living and an improved quality of life.

The role includes security checks, transportation, adult day programming and congregate dining, emphasizing healing and reconciliation within the Indigenous community.

Qualifications

  • Degree in Social Work or related human services field with specialized training or experience working with Indigenous seniors and vulnerable populations in a community setting – long term care.
  • Knowledge of chronic health conditions and gerontology-related issues is an asset.
  • Knowledge of health-related legislation and/or policies relevant to program delivery.
  • Strong knowledge and awareness of Indigenous culture and history, including residential schooling impacts, Indigenous engagement and community outreach; knowledge of an Indigenous language is an asset.
  • Experience working with urban Indigenous seniors and families in healing and reconciliation capacity.
  • Ability to deliver programming to improve health conditions (elder abuse prevention, diabetes prevention/management, chronic health management).
  • Self-motivated, able to work with minimal supervision in a team setting.
  • Excellent case/file management, time management, and ability to meet deadlines.
  • Engage people to build trust; effective verbal and listening communication; crisis intervention skills.
  • Group facilitation, team building and decision-making; problem solving and initiative.
  • Certification in Standard First Aid, CPR, AED, and Safe Food Handling.
  • Ability to work flexible hours, including evenings/weekends.
  • Vulnerable Sector Clearance Check required.
  • Must have valid driver’s license and reliable transportation; a valid Drivers Abstract required.

Job description

AREA OF RESPONSIBILITY:

The Life Long Care Support Worker’s primary responsibility is to develop and provide culturally appropriate community support services to urban Indigenous individuals that are elderly, frail, physically disabled, and chronically ill, which promote an integrated response, community participation, independent living and an improved quality of life. The program provides security checks, transportation, adult day programming and congregate dining, and Indigenous support services.

QUALIFICATIONS:
  • Preferred degree in Social Work or in a related human services field, with specialized training or experience working with Indigenous seniors and vulnerable populations in a community setting – long term care.
  • Knowledge of chronic health conditions and gerontology-related issues is an asset.
  • Knowledge of health-related legislation and/or any other policies relevant to program delivery.
  • Strong knowledge and awareness of Indigenous culture and history, with particular emphasis on residential schooling, the impacts of trauma, Indigenous engagement and community outreach; knowledge of an Indigenous language(s) is considered an asset.
  • Knowledge and experience working with urban Indigenous seniors and families in a healing and reconciliation capacity.
  • Ability to deliver programming to improve health conditions, i.e. elder abuse prevention, Diabetes prevention and/or management, chronic health conditions management, etc.
  • Self-motivated individual with the ability to work with minimal supervision in a team-oriented setting.
  • Excellent case / file management skills, stress and time management skills; ability to meet deadlines.
  • Ability to engage people to build trust and rapport; effective verbal and listening communication skills; effective crisis intervention skills.

  • A clear Vulnerable Sector Clearance Check is a condition of employment.
