Responsibilities

The incumbent provides care, treatment, and service to Home Health and Hospice patients and their families in their place of residence within their scope of practice. Functions as a member of the interdisciplinary team.

Qualifications

Experience:

1. Two (2) years LVN Experience within the last 5 years
2. Recent experience as an LVN in a Home Health or Hospice Agency preferred.

Licensure:

1. Currently licensed as a LVN in California
2. Current California driver's license, possession of automobile insurance & access to an operating motor vehicle.
3. Current CPR/BLS card approved through the American Heart Association.
